“TOPS: a reliable and simplified tool for predicting mortality in transported neonates”

چکیده
Background: Illness severity scores have been used as predictors of neonatal mortality to give prognostic information to parents about their baby,to identify high risk babies for prompt action (triage) and for standardized comparisons between neonatal units for quality assessment. Objective:To evaluatethe TOPS (Temperature,Oxygenation,Perfusion and blood Sugar) score predictive value for hospital death so as to institute an easy tool for assessing mortality index in transported neonates. Methods:A prospective study was conducted in a tertiary teaching hospital enrolling 500 transported neonates. TOPS scoring was done at admission (temperature by digital thermometer in axilla, saturation by pulse oximeter, capillary refill time in mid sternum region, blood sugar by reagentstrip) and babies were followed for outcome. Results: A mortality of 22.8% was observed. Prematurity and birth asphyxia were leading causes of mortality. Derangements in TOPS variables had significant correlation with neonatal mortality onunivariate odds ratio analysis. Hypothermia was commonly observed in transported neonates (39%).The sensitivity, specificity, positive and negative predictive values of derangements of two or more TOPS parameters in predicting mortality were 71.9%,80.8%,64.3% and 90.1% respectively. The score accuracy for mortality was confirmed (area under the ROC curve = 0.764). Conclusions: Besides being useful to predict hospital death, TOPS was a simple score that can be easily applied in neonatal units.Based on these results, we recommend TOPS score to be done routinely for all the babies at admission.

A Study on the Current Status of Neonatal Transport to a Special Newborn Care Unit
Introduction: Neonatal care in India has witnessed a revolution in the last 10 years under the auspices of National Rural Health Mission (NRHM). The major causes of neonatal mortality in developing countries are Prematurity, Birth asphyxia and Sepsis. Institutional delivery and in utero transport of newborn is the safest way to transport a sick neonate. But unfortunately preterm delivery and ma...
متن کاملبررسی فاکتورهای خطر کلینیکی ((Clinical Risk Index for babies در پیش گویی بقای نوزادان با وزن بسیار کم
Objective: Very low birth weight(VLBW) babies constitute approximately 4%-7% of all live births and the mortality in this subgroup is high, contributing to as much as 30% of early neonatal deaths. Some scoring system like the Clinical Risk Index for Babies(CRIB) Score and the Score for Neonatal Acute Physiology (SNAP), for assessing the risk of mortality frequently utilized in newborns....
متن کاملDoes transport affect the incidence of intraventricular hemorrhage in preterm infants?
Objective: Preterm infants are prone to intraventricular hemorrhage. They may suffer from significant morbidity and mortality particularly in those with high-grade hemorrhage. Our objective was to determine the incidence of intraventricular hemorrhage in premature infants transported to a tertiary center compared with those delivered at the level III facility. Methods: We evaluated all prematu...
متن کاملSimplified Symptom Pattern Method for verbal autopsy analysis: multisite validation study using clinical diagnostic gold standards
BACKGROUND Verbal autopsy can be a useful tool for generating cause of death data in data-sparse regions around the world. The Symptom Pattern (SP) Method is one promising approach to analyzing verbal autopsy data, but it has not been tested rigorously with gold standard diagnostic criteria. We propose a simplified version of SP and evaluate its performance using verbal autopsy data with accomp...
متن کاملA comparison of CRIB, CRIB II, SNAP, SNAPII and SNAP-PE scores for prediction of mortality in critically ill neonates
Abstract Background: Clinical Risk Index of Babies (CRIB), Score for Neonatal Acute Physiology (SNAP), an update of the Clinical Risk Index for Babies score (CRIB II) and Score for Neonatal Acute Physiology - Perinatal Extension (SNAP-PE) are scoring devices developed in neonatal intensive care units. This study reviewed these scoring systems in critically ill neonates to determine how well th...
